Initial Comments:
An off-site follow-up survey conducted on August 16, 2021 found that Masonic Village Hospice had corrected the deficiency cited under 42 CFR, Part 418, Subparts A, C & D, Conditions of Participation: Hospice Care. The deficiency was cited as a result of a hospice state re-licensure survey completed on June 25, 2021.
